Adding Incidence to Evidence: Treatment of Symptomatic Apical Periodontitis in A Maxillary Canine With 2 Separate Canals (Vertucci Type IV)

The knowledge of root canal anatomy and its variations is a key for endodontic diagnosis and a successful root canal treatment. The current case report discusses the endodontic management of an anomalous maxillary left canine with a rare Vertucci Type IV root canal configuration (reported in only 0.8% cases). A patient reported with the chief complaint of pain on mastication in left upper jaw. Maxillary left canine was diagnosed with symptomatic apical periodontitis associated with pulpal necrosis. Angulated IOPA radiographs suggested Type IV canal configuration. Endodontic treatment was initiated and the additional palatal orifice was located with the aid of magnification and DG 16 explorer. Working length was determined and shaping was done with ProTaper Gold rotary files. Post endodontic restoration was done using fibre post and a core build up restorative resin which was followed by placement of crown.
